Single-Page DEA Form 222 Now Required for Schedule I and II Controlled Substances

Effective October 30, 2021, all DEA registrants will be required to use the new single-page DEA Form 222 to purchase, transfer, or dispose of Schedule I and Schedule II controlled substances.

The old Form 222, a carbon-copy triplicate form, will no longer be accepted after October 30. Any extra unused copies of the discontinued three-page form must be sent back to the DEA for proper destruction.
